 First video  visit will be one hour comprehensive evaluation in your first month of membership. 

 

Second video  visit will be one hour where Dr Cruz will  review lab results; outline a plan for preventive measures  incorporating standard medicine, lifestyle modification, and preventive with holistic approach incorporating osteopathic philosophy.

 

The follow up visit  will  be 45 minutes video visit consisting  of: 

reviewing your lab results in detail if indicated at that time, discussing your progress, and  adjusting current plan to assist you to reach goal plan.  

 

There will be a total of 7  video visits total per year. Two  one hour video visits the first month of your initial date and 5  video visits that are 45 minutes scheduled every other month. 

At the start of the renewal year, the schedule of video visits repeats.

 

Members have access to their patient portal during the duration of their membership. 

 

 

After each video visit, members will receive a detailed  post plan via  email.
